Gilda: Urgent support needed: Neurological despair, extreme kindling, and adverse reaction to Mirtazapine

Thank you truly from the bottom of my heart for the time you are giving me. I am honestly desperate because I have an internal 'engine' of agitation running inside me that won't turn off. I have pains everywhere, my head feels 'electrified,' I have stomach and abdominal pain, and now I’m even breaking out in acne on my face. I have tinnitus so severe it feels impossible for a human being to endure, and I am also stuck with orodispersible mirtazapine. I read that at the dose I am taking, it acts as a very potent antihistamine, so it is ruining me.

I know that stability is important, but by dividing the dose this way, I am unable to achieve it. I imagine that for the time being, I must continue with the same method of intake; otherwise, I risk destabilizing myself even further. I want so badly to get off it, but I’ve seen that I react poorly to changes, yet at the same time, the drug is hurting me.

Everyone here thinks I am depressed. I explain what I am going through many times, but they don't believe me. I feel so alone in this battle. I live by myself, and I have to work. Every passing minute is a tragedy. I cannot afford not to work; otherwise, I risk losing my job. Currently, I am working from home, but from Monday, I have to go into the office. I even explained my situation to the company doctor, but she just wrote on the certificate that I am 'anxious' and have tinnitus.

No one believes what I am going through. No one believes that decreasing the mirtazapine dose by such a tiny amount could make me feel this ill. I cry every day. I am a woman who has a home, pays a mortgage, and has to work and cook for herself. Everything I do requires a superhuman effort. I’m sorry for the vent.

I feel you, @Gilda I had to work through my withdrawal because my job carried my family's insurance. It was tough but I made it. Some days I couldn't do much more than listen to a calming audio over and over, but I kept at. Eventually, work became somewhat therapeutic because it forced me to focus on something other than my symptoms and my anxious thinking.

I found the same as you're experiencing; even family members who were supportive couldn't fully understand what I was experiencing. It has to be lived to be understood. But you're not alone--we're here to support you!

Hey @Gilda yes many of us on here have had our lives turned upside down by this nasty drug. I am sorry you are having to deal with it too. This is too common a story, you are not alone Gilda. I am v

Hi @Gilda. I feel you very well. I have stopped Mirtazapine March 20 at 1,5 mg which I have tapered since June 2025. As at a low doses this drug acts as a histamine, a big crush became after stopping it. The hellish things are the cortisol rushes + sweats + dépression, I cannot sleep more than 1-2 hours. As you, I have internal agitation, restlessness, stomach and muscle pains. In plus a horrendous anhedonia and nausea. But we have to support each other and hope in the body's intelligence to heal.

Unfortunately, my story is extremely complicated. From December 2024 to March 2025, I took Xanax (Alprazolam) intermittently during a very difficult period. In the last weeks of March 2025, I started taking Remeron (Mirtazapine) at 15 mg. One day, I had a panic attack at the supermarket; I rushed home and took a Xanax. The panic subsided immediately, but I developed tinnitus. Terrified, I stopped everything cold turkey. I felt terrible, with extreme agitation, and I had to continue taking Xanax just to be able to go to work. Two weeks later, I stopped everything again. I still had some anxiety, but the tinnitus was getting louder every day.

In May, an ENT specialist prescribed Rivotril (Clonazepam): 0.8 mg in the evening and 0.4 mg in the morning. The tinnitus disappeared. However, it returned in July, so I decided to taper off the Rivotril by reducing it by 0.1 mg every one or two weeks. It was difficult: terrible stomach and abdominal pain, and excruciating muscle pain, while the tinnitus continued to increase. When I reached the final 0.2 mg, most of the symptoms were finally fading and the tinnitus had decreased. One Saturday, in a hurry, I didn't use earplugs while drying my very long hair, and the noise of the hairdryer triggered a massive spike in my tinnitus. The ENT suspected acoustic trauma and prescribed cortisone, which only made the situation worse.

I continued to taper the Rivotril until I finished it, but at the beginning of December 2025, I crashed: fever, extremely loud tinnitus, clogged ears, hyperacusis (all sounds were amplified), and reactive tinnitus (the sounds changed and increased in volume in response to noise). Another ENT thought it was catarrhal otitis and prescribed cortisone aerosols, but it had no effect and I became increasingly agitated. In January, vertigo began; the ENT tried maneuvers for displaced otoliths, but nothing worked.

In February, I consulted a psychiatrist who started a trial-and-error therapy with various drugs: one night Remeron (Mirtazapine) 7.5 mg, the next night Zyprexa (Olanzapine) 2.5 mg (which caused terrible nightmares), the day after Zoloft (Sertraline) 25 mg (extreme anxiety), and two days later Cipralex (Escitalopram) 1 mg (which made me feel physically ill and increased the tinnitus). The following day, I was hit by an excruciating visceral anguish that lasted 3 days. I remained depressed and tried to reintroduce Remeron at 7.5 mg, but the tinnitus worsened to the point that I had to stop after two days.

From there, I hit rock bottom: two weeks later, I had unbearable visceral anguish, depression, panic attacks, and tachycardia. I realized it was likely subjective akathisia. After a month, I resumed Remeron at 7.5 mg and then 3.5 mg, and I felt slightly better. However, the psychiatrist advised me to reintroduce Rivotril (Clonazepam). One morning I took 0.1 mg and felt fine, but the next morning I had severe back pain. I took another drop and the pain intensified. Scared, I stopped the Rivotril and increased the Remeron dose to 7.5 mg. Since then, it has been a total disaster: unbearable tinnitus in my head and ears, a strange sensation as if my head were echoing, panic attacks, extreme agitation (like being in a constant state of shock), and stabbing pains everywhere.

I believe my nervous system was sensitized by the Rivotril taken for 8 months and then battered by the other drugs I took. I think those isolated doses of Rivotril caused brain activation on an already unstable system, triggering kindling. No doctor understood that these were withdrawal symptoms from the Clonazepam I finished in December 2025 after 8 months of use, which is why they kept giving me more drugs. Two weeks ago, I reduced the Mirtazapine to 5 mg but was forced to bring it back up to 7.5 mg. Then, last Saturday, I reduced the Mirtazapine dose to 6.8 mg.

Unfortunately, now it’s not just about withdrawal. It is something serious. I am in so much pain. I feel no pleasure in anything. I can't watch TV. I feel like an alien. The world outside goes on; people laugh, and I feel like I'm dying inside. I feel no joy. I have apathy, agitation, anguish, pain in my back and buttocks, and tinnitus that isn't normal—it’s high-volume noise in my head and ears. My head feels strange, heavy, 'electric,' and detached from the rest of my body. My brain is no longer functioning. The Mirtazapine is making everything a thousand times worse, and I can't even get off it. Plus, at a low dose, it’s a powerful antihistamine. I am falling apart.

Medication History

  • January – March 2025: Use of Xanax (Alprazolam).

  • Late March 2025: Mirtazapine 15 mg for only two weeks.

  • May 2025 – December 2025: Use of Rivotril (Clonazepam) (8 drops in the evening, 5 drops in the morning).

  • December 1, 2025: Discontinuation of Rivotril (Clonazepam) after a 4-month taper. Experienced various physical pains but no depression or agitation. Developed tinnitus. Since then: chronic agitation, vertigo, diagnosed by psychiatrists as "automated anxiety," and persistent tinnitus.

The Final Breakdown (February 2026)

After discontinuing Rivotril (Clonazepam), I suffered trauma from "polypharmacy," triggering a violent reaction:

  • February 6–16: Rapid rotation between medications (Mirtazapine, Olanzapine, Zoloft, Cipralex).

  • February 17: I decided NOT to take the dose because the tinnitus had become unbearable.

  • February 17 – March 22: Total discontinuation. The abyss: subjective akathisia, debilitating panic attacks, constant chest tightness.

Chaos and harm caused by dosage variations in March:

  • March 22–23: Reintroduction of Mirtazapine 7.5 mg. Subjective akathisia improved, but severe chest pain appeared.

  • March 24–27: Dosage reduced to 3.5 mg. Reintroduction of Rivotril (Clonazepam) (1 drop on March 26 and 27). It was after this reduction that physical pain, immediate and violent, exploded.

  • March 28: I discontinued Rivotril (Clonazepam) and returned to Mirtazapine 7.5 mg. I felt extremely ill for a week with a "strange head" and pain. Then, slowly, I stabilized slightly. Tinnitus decreased. I have good days, but I still have a lot of agitation.

Recent events:

  • April 18: Reduced Mirtazapine to 5 mg, but was forced to return to 7.5 mg the following day. Since then: strange sensation in the head, back pain, violent tinnitus.

  • April 25: Reduced to 6.8 mg of Mirtazapine. Worse than before: severe back pain, depression, insomnia, stomach pain, muffled ears, tinnitus in the head. Increased agitation, visceral akathisia appearing throughout the day, headache.

  • Currently, April 29: Reduced Mirtazapine dosage to 6.8 mg

CIAO@GildaTi capisco benissimo.

@skamen Thank you for sharing your experience. I didn’t quite understand how long you took Mirtazapine for and at what dosage. Unfortunately, I am still taking it and I feel terrible. However, I can't stop taking it because I feel even worse when I try.

Did you feel unwell while you were tapering down, or did the symptoms hit you once you had completely stopped? Also, how did you manage the reduction? Were you using standard hard tablets or Orally Disintegrating Tablets (ODTs)? How did you measure exactly how much you were taking? I am stuck with the ODTs, which just crumble when I try to cut them. I really believe that these low doses are harmful. Please let me know how you handled this.

@mstimc60 Thank you so much. It is so important for me to talk to someone who truly understands. I have been a victim of doctors who only cared about profit. The symptoms I had were actually withdrawal from Rivotril, yet they diagnosed me with conditions I never had. Unfortunately, back then, even I didn't know what was happening to me. Now I know everything, but I find myself in this disastrous situation. If I hadn’t taken anything else since December 2025 after stopping Rivotril, I wouldn't be in this state now; I might even be healed.

Instead, I am in a situation where every minute is just about survival. This isn't living. I am suffering immensely from depression and extreme 24/7 agitation—it feels like an engine that is constantly running and nothing can turn it off. It’s subjective akathisia. I feel visceral anguish, pain everywhere, violent tinnitus, and a strange, heavy head that feels detached from my neck, along with many other symptoms. It is too much for a human being to bear.

I can’t even sleep. If I do manage to fall asleep, I wake up every two hours. I fall asleep very late and struggle to get up in the morning, and the moment I stand up, it all starts all over again. Facing the day is exhausting. I used to be a cheerful person, I had many friends, I went out. I only started taking Xanax in 2025 because I was going through a particularly stressful time and couldn't sleep. Then I added Mirtazapine for the same reason, but I wasn't depressed.

Then, an ENT ruined my life by prescribing Rivotril for tinnitus that was nothing compared to what I have now—tinnitus that was actually caused by the Mirtazapine (a connection I am only able to make today). When I think back, it was even starting to decrease then. Now, I see no way out. I am trapped between this hyperactive nervous system and the Mirtazapine, which is having a paradoxical effect on me, yet I can’t decrease it because I feel even worse. I’m sorry for the vent, but I am truly desperate.

It took me three years to go through withdrawal and arrive at what I consider recovery. During withdrawal, I defined my feelings as like being a trapped animal--all energy and agitation and no way to let it go. Its is awful. One way to cope is to burn off the physical energy--even if its by walking around in your home if you can't go outside. Recovery will come but on its own timeline. I know things seem awful now but you will get through this.

I'll let other members more familiar with Mirtazapine give you a definite answer, but if you believe its harming you, it would be better to start a slow taper than to allow it to continue to harm you. Looking at your bio, I suggest you stay at your current dose to let your nervous system stabilize a bit before doing another reduction. See if you can start practicing simple coping techniques to deal with your symptoms and slow the negative thought cycles. Be kind and patient with yourself.

Dear @Gilda, we are in the same boat. I have been 14 years on Paroxetine which have thrown me in hellish anhedonia in February 2024. The psychiatrist started to prescribe me AD after AD, I did 70 rTMS sessions, 8 ketamin infusions... Nothing worked, in opposite, these constant changes of drugs and doses have broken my CNS. In March 2025 he cold turkey-d me from 3 psychotropics and told me to remain on the 4 one, Mirtazapine. From this moment my Living Hell started. During this Hell I have tapered Mirtazapine and have reduced the last 5 mg from September last year to March 20 this year. I never been stable even I have done a very small cuts. But because my brain was already so sensitized and because of the brain injury after the CT. As you, I cannot read, cannot watch TV, nothing distracts me, I have inner akatisia, constant terror, cannot sleep. All the night I have an awful cortisol spikes, hot flashes, the depression kills me. I am no more an human being, I am reduced to a nut shell. I am starting my month 14 of Hell. After the last dose of Mirtazipine every symptom increased x1000 and the cortisol spikes became unbearable. I am so sorry that we suffer so deeply.

I am so sorry to read this. You are right Mirtazapine is an antihistamine more potent at low doses. It also messes with many different receptors, we call it a dirty drug as a result. Many of us have been harmed by it.

Did you take these ODT tablets before when you were on Mirt or did you only just start them in March?

I know, and I am sorry. This is so common. People just can't understand. It is both frustrating and leaves us feeling invalidated and lonely. We all get it. You are not alone here. Feel validated.

@Gilda

Ive read your others posts now. I am really very very sorry to read all this.

I think you are spot on with your assessment of what has happened to you. Unfortunately it is a too common story.

It is now very hard to know what you should do now. As you have said, this is more than WD. This is a nervous system injury.

Often doing nothing is the right thing. At least in the short term. Gives the brain less to cope with whilst it works hard to repair and adapt.

If you feel Mirt has gone paradoxical on you, then perhaps tapering off it is the right move. The longer you are on it, the more dependance you gain, the harder it is to come off it due to WD effects. There is no rule book here. It is just a case of feeling your way through and listening to your body. It may be that being off Mirt is what you need, so your body can finally have some peace to heal.

@Finni Read my story here; it is similar to yours. I also had to take many different medications, and now I am stuck with Mirtazapine. I believe it is harming me, but I can't get off it because I feel even worse when I try.

I know how hard it is to know what to do! I’m feeling very activated by mirtazapine myself and tried to reduce by 1% last night. I’m trying to figure out if it was a mistake or not because I feel worse now.

If I were you, I would hold for at least 2-4 weeks and see if anything starts to settle down.

Hey Gilda, I'm sorry about your experience with those drugs.

I just want to add some mindset things for you to help you out with your experience.

I would highly recommend doing some mental works and working on your mindset.

All these symptoms are temporary and will end at some point, but you will have them now.

You have 2 options:

  1. You let the symptoms toy with your soul, which will make time feel 10x slower and cause much more suffering.

  2. You accept all your symptoms, and when they are very severe, you surrender to them and let them do as they want until they 'get tired', when you do those things, and don't get attached to the bad emotions and symptoms, time passes 10x faster and you also help yourself heal quicker.

I will leave some coping topics below:

So bene quanto sia difficile sapere cosa fare!

How do you know the exact amount of Mirtazapine you are taking? I have orodispersible tablets that I cut and weigh, but I know perfectly well that the actual dose is different every day. I feel trapped. As for Rivotril, I had reached 1.3 mg (which is 13 drops)

@Chippy@LostinCanada "Hi, I’m taking orodispersible Mirtazapine. I would like to switch to regular hard tablets. I believe that even though I use a scale, the drug is not evenly distributed within the tablet, and when I cut it, it crumbles. This might be why I can’t stabilize. Maybe I should switch to the hard tablet and dissolve that one instead.

Also, I’d like to know why there has to be a gradual transition to the liquid form. The active ingredient is the same. Since my current tablet is orodispersible, could I switch directly to liquid by dissolving a regular tablet? I noticed that when I dissolve the orodispersible one in water, it forms tiny beads, and the drug is supposed to be inside them.

My orodispersible tablet weighs 0.311 g for 15 mg of drug. In contrast, the hard tablets weigh 32 mg for 30 mg of drug. If I crush the orodispersible ones, I don't know how much actual drug I’m getting in the powder because almost half of it is filler. I need to take 6.8 mg. If I crushed the regular tablets, there would be much less filler—only a 2 mg difference—compared to the orodispersibles where half is filler.

I am desperate. Please help me. I don't know if I can switch to regular tablets while I’m currently on the orodispersibles. My nervous system is in crisis. I am feeling really, really bad."

@Gilda you can switch if you want to. Switching can be destabilising which is why we recommend crossing over rather than a hard switch. I can’t see any reason why you couldn’t crossover to a liquid made from the standard mirt tablet, from the odt tablets in dry form. Obviously you are not very stable right now so it’s not ideal to switch but I understand you want to have a method you feel is more accurate.

We can help you switch if you want with the maths and so on just let us know.

Thank you, I can manage the math. However, I’m not sure if the body perceives the switch from orodispersible to regular hard tablets. I don't know if the drug reaches the system in the same way. I'm worried about how my body might react to this change.

It seems that you could use the odt tablet and dissolve them in a certain amount of water and reduce from there 🙂

I use a scale and file down my tablets, but if I had the orally disintegrating kind, I would use water to reduce

According to my research, the drug is evenly dispersed throughout the tablet. To get accurate dosing, the dose needs to be weighed.

why there has to be a gradual transition to the liquid form

This is because, each manufacturer has variability in the quality of the drug and filler which affects absorption. Transitioning to liquid also affects absorption...going slowly prevents destabilizing.

It has been carefully mixed and pressed into tablets with highly mechanized machinery that is regulated and tested. All of us are dry cutting are taking pills that are made this way. My pills weigh 45.6 mg for 3.85g.

Once you cut your orodispersable pills, any leftover bits should not be stored but disposed of. The drug quality lessens with air exposure.

Thank you. The problem with orodispersible tablets is that when I try to cut them, they are not precise. The cut is inaccurate and I lose powder. It’s different from cutting a normal tablet. Then I have to remove a small piece and it crumbles. And if I weigh what I’ve cut, I can’t get the exact weight I want. Then the piece keeps crumbling. It’s a nightmare. And if I switched to regular tablets, do you think it would be a problem for my nervous system?

Who told you that? I have orodispersible tablets, and I assure you they don’t dissolve. They remain as little pellets that contain the medication inside. These pellets stick to the sides of the glass when I stir, and many others settle at the bottom. I assure you that regular tablets dissolve better in water. I’m really struggling because I have the orodispersible tablets.

This is why just cutting is not accurate and weighing has to be done. I assume you are weighing to get your dose of 6.8 mg. Instead of cutting, would shaving with a one side razor or filing with a metal nail file work better?

Any switch if not done slowly and carefully can be destabilizing. Does your manufacturer also make regular mirt pills?

If this is the case...I can see how accuracy could be a problem if you are cutting and weighing. I do not know much about this type of pill but it definitely complicates the situation. The pellets/spheres should dissolve because they are made from sugar and starch.

@Chippy did you know the following? To me this means, cutting and weighing could not be accurate due to the matrix.

"In the Remeron SolTab formulation, the tablet structure is essentially a delivery vehicle for these tiny drug-loaded beads. Here is how the drug is distributed:Drug-Loaded Pellets: Each pellet consists of a core (the sugar sphere) that is coated with a layer of the active medicine, mirtazapine.Taste-Masking Coating: A second protective layer—usually a polymer like Eudragit E100—is applied over the drug layer. This coating is crucial because mirtazapine is extremely bitter; the polymer is designed to stay intact in the mouth (at neutral pH) so you don't taste the drug, but it dissolves quickly once it hits the acidic environment of your stomach. The Matrix: The rest of the tablet (the "matrix") is made of fast-dissolving materials like mannitol and crospovidone. When you place the tablet on your tongue, this matrix disintegrates in seconds, releasing the individual pellets so you can swallow them easily with your saliva."
