Jazzjunkie84: My journey with misdiagnosis, covid, duloxetine

Featured Replies

Hi to everyone who is unfortunately brought together here by this process. You all keep me going in the hardest moments. Thank you.

I want to introduce myself as I am finally joining to get support on the final stages of my duloxetine taper. I am in a unique(?) situation in that I was actually experiencing what most here would describe as severe withdrawal PRIOR to being on this med from a worsening of my post covid condition that was misdiagnosed as “panic disorder.” I was unfortunately not very well verses in nervous system issues or long covid at the time and couldnt advocate for myself.

A very brief backstory.

My only encounter with meds before 2025 was situational. I took low dose amitytiptyline prn after a gallbladder surgery in 2013 for sleep. Maybe one pill a week. No issues and never needed it again. An abusive ex partner also tried to convince me I needed meds and so i trialed latuda/citalopram in 2013 for about 2 weeks and quit because it was soo awful. No issues withdrawing and I did not take any other meds besides the ocassional ibuprofen until 2025.

Post covid/vax in 2021 I developed some atrange symptoms like floaters, dizzy spells, episodes of brain fog, but no tests showed abnormalities and they always went away so I ignored them. Life was great otherwise. I was sober, highly trained athlete and doing very well in my phd. In March 2025, I suddenly developed akathisia, insomnia, severe brainfog, shaking, tachycardia, lost weight, exercise intolerant, mouth sores, reactive hypo (which I would not know until i paid for a cgm out of pocket a year later) etc. Gp blamed it on anxiety/phd work, had me try cbti and counseling which actuallt made me worse (positional changes worsen POTS so naturally getting up if i couldnt sleep just wound up my nervous system more!!). I was in canada at the time and refused a sleep study or neurology referral because “I didnt fit the profile.” I was basically forced to try an antidepressant. Started with nortriptyline for 2 weeks which changed nothing except giving me blue hands and chest pain, I went back to the er and told to cold turkey. With no improvement and suffering greatly from the insomnia and constant hypnic jerks, i went back to the er and was given cymbalta after refusing the other types of antidepressants ans benzos. They refused to consider treating me for anything other than psychiatric disorders so I had no choice in my mind.

I returned to the states, decided the medication was not helping at all and began tapering as per the 5percent rule by bead counting. I cant say for sure whether some of the brain fog and easing of my akathisia was from dulox but for a solid six weeks after starting my blood sugar was dangerously low pretty much every hour, sleep was worse, horrible sweating and dry mouth, pots symptoms worse, terrible sinus infections and bowel changes around my period cycle i never had before etc.

I got a sleep study done and the doc immediately suspected narcolepsy and or limb movement disorder but cant run the test until the duloxetine is finished. Offered a cross taper to prozac but I declined.

My taper went pretty well from June2025 to January of 2026. I would usually have a few hours here and there of odd depressive thoughts out of nowhere or weird muscle twitches and some hypnic jerks but nothing debilitating. Because I was concerned about my sleep study referral running out I tried a bigger cut in dec-january. I paid for that with a few weeks of on and off severe symptoms - mainly the return of the akathisia and insomnia and intense doom and food issues. Thankfully they resolved with a hold and I went back to small cuts again every other week.

I was feeling pretty great in the 12-7mg window. Still having some issues with sleep and energy but feeling pretty normal again in other ways. Could finally eat some tasty foods and enjoy light exercise for the first time in a year.

I again attempted some faster cuts this summer as I am a school teacher and cant afford to deal with withdrawal during the school year. I made some mistakes with my schedule bc of brain fog and accidentally made a few cuts in a short time (see my signature). I have basically relapsed to my march 2025 state with small windows of relief midday. I had to present at a work event and somehow in my fog forgot my pills so had a further blip last week using a different brand i acquired thru emergency but the bead count was different so it had me at a temp increase before I got back home.

Because of the cyclical nature of my symptoms intensifying at night and in early morning before my 7 am dulox pill, i am quite certain I am just experiencing cyclical withdrawal. I have begun transitioning to split dose by moving half of my beads to a second pill back an hour every day until i have a 12-12 split.

The hardest part for me right now is believing I am in withdrawal and not some reemergence or worsening of my long covid symptoms. I know I have to get off this drug either way for the sleep study, which will likely be positive, but given my condition prior to the drug, it is very scary and nearly impossible to understand what is happening or how to deal with symptoms. As many of you know, the medical system is nearly useless so it is not only difficult to explain my situation to my work supervisors but also impossible to hope for any kind of disability/leave support from a doctor.

Hoping some others here are in the dual longhaul/withdrawal space to help commiserate or maybe provide insight. I remain confident that my feeling great throughout tjr taper (much better than when I was on the full dose) is reason to believe I am healing from withdrawal and not relapsing.

Again I had something very similar, but not akathisia or tachycardia, i got extremely ill but because i was in panic cause i felt like my body was slowly dying (at some point i was bed bound), they just decided it was all mental and i made it up out of nowhere, and just gave me a million drugs in a bed bound vulnerable state, which is how I got nervous system injured from these drugs.

sounded like you tapered well in that period mostly (besides the big cut)

did you start having those issues only after tapering in a wrong way ?

You are healing, we all are, it's important to believe that you will get out of this, as it would help you and your body ❣️

This is a temporary state, our feelings and symptoms can lie to us, it's important to pick and choose our thoughts, and choose which ones to believe and which not, believing you will never heal won't help you and will hinder you.

In the mean time this is what I would do if I were you:

-Stay Hydrated

-Eat a good clean whole food diet, avoiding processed foods and sugars.

-Stay away from caffeine, alcohol and antihistamines and any other psychoactive substances.

Supplements are to be avoided initially, as they often irritate our nervous systems. Try to get your nutrition through food.

Down the line, you may wish to introduce Fish Oil and Magnesium. Those who can tolerate them have found them calming to the nervous system. Be careful try one at a time and start low and see how you get on should you choose to try them.

A daily walk is very important as much as you can manage, if you can manage it. Gets you your fresh air and Vitamin D does wonders for your mind!

Current Supplements: Night: 6mg melatonin and 133mg magnesium glycinate. Taking Omega 3 as needed to calm dyskinesia or over stimulation (Lately been reacting more negatively to supplement Omega 3).

Current Medications: Mirtazapine, Lasea (lavender oil) before bed.

------------------------------------------

Tapering: Mirtazapine 15mg, (went compounded) 13.5mg 08/May/2025, 12.1mg 10/July/2025, 15/July/2025 15mg (half tablet), 26/July/2025 14.35mg (moved to dry cutting method) ), 03/Aug/2025 14.6mg, 24/Nov/2025 14.47mg, 29/Jan/2026 14.35mg

Note: Had a lot of issue with degradation with different cutting times and compounded pharmacy which caused withdrawals and a more sensitive nervous system.

Today I feel good physically. I took a 5mg orexin antagonist (I had been doing this regularly but had a few days recently I tried going without or cutting in half because they are so expensive).

Trying so hard not to let myself get excited about anything today. I have learned the hard wwy that when I feel good and I let myself “forget” I am ill, if I feel ANY excitement (nervous system arousal) from music to a fun conversation or any physical activity that I actually get endorphins from, I do NOT power down and will not sleep and then have severe dysautonomia problems for days. It is just SO cruel.

I am also logging my other medications. I currently take 4 beads of cym at 7 am and another 3 at 9:30 (aimint to shift to split 12-12 dose over time)

-metoprolol 12.5mg 7 am and 7 pm (had been off march 2026 but reinstated July 5 because of akathisia)

-ketotifen .5mg 7 am and 7 pm (I know anithistamines arent suggested but ive been on this regularly since 2025 since cymalta made me have allergy type reactions and it has been the ONLY thing to provide relief and reatful sleep until recently. I dont think changing meds is safe right now so im sticking with it)

-cholestyramine with lunch bc I do not have a gallbladder

Supps:

-iron w/buffered vitamin c (i am deficient no way around this one)

-fish oil

-vitamin d and k (have been taking for over a year no problem)

-zinc

My first split dose was friday and so far I am going through thr typical pattern i experience with medication changes. I felt iffy physically the day of. I felt like my old self, almost a little manic the past two days. Today i feel narcoleptic and constsntly on the brink of a sugar crash. Brain fog is bad, concentrating makes me irate because its too much.

I am promising myself i will stay the course and not make any changes for two weeks. No weird food or diet/food window changes, try my best to maintain sleep schedule but grace to nap if i need to because strictly sleeping only at night seems to do nothing but give me more symptoms.

I really hope i stabilize. Its been over a month now. Sometimes i wonder if whatever was going on becore i got slapped in this med is gunna come back. I wish i knew for sure it was the med withdrawal and not my condition reemerging as it were. No choicr but to come off anyway since i HAVe to do this sleep study.

Thanks for the update. I agree hold the line on your split dose. It can feel like a drop especially if you jumped times rather than moved gradually over. Holding is often the key to stablising. The body doesn't like changes very much.

Trust this is WD. Remember you were lied to. There is no chemical imbalance or condition that needed curing. Just a human dealing with life and enviroment in a human way. These drugs harm and then try to hide the harm by allowing us to blame ourselves. You've got this.

Thanks chippy.

I had a horrendous night last night. Weird twighlight reatless fake sleep (somehow with vivid nightmares). I woke up with the inner aksthosia around 5. Noticed my cgm said my glucose was low. I ate some cornstarch and nut butter balls and went back to sleep about an hour later that actyally had my feeling physically refreshed somehow so i am thankful for that.

Feeling the tightness in my chest/adrenals and on edge.

The hardest part is that I know something IS wrong though. I was having these WD like symptoms before ever been put on the meds, especially the weirs half awake half asleep drug state. Thats the reason im beinf screened for narcolepsy. :/ I am just trying so hard to survive and not lose my job so i can get the dang test. It is looking pretty grim.

I personally think there are many conditions that mimic AD withdrawal. I have noticed my husband's fibromyalgia has many similar symptoms and COVID seems to as well. My husband also has thyroid issues and was put in too high a dose of Synthroid which presented itself with anxiety, shakes, sensitivity to noise, brain fog, etc. Our CNS runs the show and when it is disrupted, all we can do is allow the best remedy...time, acceptance, making an environment for healing, lessening stress, etc.

Adding drugs to your already fragile CNS has definitely complicated things.

Just my thoughts. ❤️🙏

@LostinCanada You pointed out something very important. I want to say, what if condition called WD is just the same condition or mechanism like something else, just named differently? Based on the symthoms the same person can be diagnosed by different doctors separately with fibromyalgia, long covid, CFS, multiple chemical sensitivity, IBS, functional neuropathy... Just diagnosis by itself can be in reality refereed to the same underlying mechanism...

2009-2010: Sertraline (50 mg)

2011: Escitalopram (10 mg)

2009-2011: tried Olanzapine (5/2.5mg), Quetiapine  (50/25mg) and Aripriprazole (low dosage)

CT Escitalopram, being free for several moths

2012-2016: Fluoxetine (60 mg)

2016-2017: Venlafaxine (75 mg)

2016: just tried Duloxetine and Maprotiline

2017-2018: Mirtazapine (30/45mg)

2012-2018: Bupropion (300 mg)

2014-2018: Lamotrigine (50/100/150 mg) CT Bupropion, tapering Lamitrogine and Mirtazapine (practically CT, fast tapering)

Too me, I would look at the timing. Also, I think once we have a sensitized CNS it is more vulnerable and therefore other factors can multiply the issues we are experiencing.

Doctors love to diagnosis with a view to prescribing....more drugs can cause more problems. We all have to weigh the risk/benefit.

I am not a medical professional. My comments are based on my personal experience and information on SA.

And because of bureaucratic reasons. To put a case of a patient into one category. Just to do something.

It's a great approach, but it shows to us just the opposite. You can get WD symthoms months or years after quitting. I know, you guys will interpret is as adaptive process of CNS and so on...

2009-2010: Sertraline (50 mg)

2011: Escitalopram (10 mg)

2009-2011: tried Olanzapine (5/2.5mg), Quetiapine  (50/25mg) and Aripriprazole (low dosage)

CT Escitalopram, being free for several moths

2012-2016: Fluoxetine (60 mg)

2016-2017: Venlafaxine (75 mg)

2016: just tried Duloxetine and Maprotiline

2017-2018: Mirtazapine (30/45mg)

2012-2018: Bupropion (300 mg)

2014-2018: Lamotrigine (50/100/150 mg) CT Bupropion, tapering Lamitrogine and Mirtazapine (practically CT, fast tapering)

Youre very right about that. I see so much overlap between long covid and wd and overtraining syndrome and burnout because of the ans changes. I think the “solace” if one can say that when it comes to wd is that the ans disruption was caused by a foreign chemical and then again when the chemical was changed. In my case i do have that but on top of some other reason for ans disruption, which if it is something like a sleep disorder or subclinical thyroid issues or viral persistence, will continue as long as the disrupting issue goes unresolved. Unfortunately for me the sleep study wont happen until i can get off this stupid drug.

Keep being patient when it comes to stabilization because in my experience it can take many months before you finally start to see postive effects. It's like a snowball effect, at first it needs to build up until its noticeable.

Plus when ur in it, because of how the symptoms change all the time, its hard to see the overall trajectory when you are too busy living day to day.

Thank you so much friend.

I am scared to type it and jinx myself, but I think i am coming out of it. I have had good sleep for three days. I had an awful brain fog and neuroemotion day friday until 4 pm it just evaporated and I have felt like my normal self since. Still being careful about carbs during the day because my reactive blood sugar will flare me up. Im still being careful about consistent bedtime (and taking my melatonin and dayvigo until i have a solid full week of sleep). I am only doing gym 3x a week on tje lightest weights possible and not pushing my hr up too long. I have noticed that bracing my core realllly aggrivates my symptoms for a while - interestingly the valsalva maneuver is a diagnostic for autonomic problems.

Thanks so much for checking. <3

In general still doing better than before.

I am frustrated because since hitting my luteal phase, I am having issues with what feels like cyclical withdrawal again prticularly at my nighttime dose. I wake up many times between 4-6 am (my pills are 7 am and 7 pm) with jerk movements, panic in my chest, hungover feelings. My blood sugar is plunging more.

I have been at 7 beads for over a month now and almost 2 weeks at the split dose. Part of me is worried that especially once i go lower, there will be no way to avoid kindling my system with cyclical withdrawal. It feels like i metabolize the drug really quickly or something.

I know the chw mods would never recommend hopping off and i certtainly dont plan to hop off right now, but i do wonder once i get to like 4 beads or something if ill be suffering either way.

The other side to this coin is that i dont tend to have symptoms of the drug wearing off comung up to my 7 pm dose. I do wonder if my uneven split (4 beads in the morning and 3 at night) is contributing or if my symptoms in the morning more come from cortisol etc in the early mornjng hours.

Sorry just now responding.

Sometimes (mainly when I do too many tricep dips) i actually get popping in my sternum area. But otherwise no. Its a sensation of an organ or my blood vessels squeezing. It always comes with a shaky feeling so I think of it as part of my adrenal response.

Ok going on 72 hours keto and all symptims dialed down to 1-2. I still feel kinda “giddy” and restless after eating but sleep is much improved and havent felt crazy hungover in the mornings. I feel tired but it feels like an uncovering of what the adrenaline had covered up. I do not want to stay low carb indefinitely as i am worried about longterm metabolic changes. But i am treating this as an elimination diet for now. If it works for two weeks ill try decreasing a bead and seeing if that helps with the withdrawal i have been getting.

Update keto effect sadly didnt last. I felt ok ish but started to have the stomach churning. Even had some glucose lows overnight despite essentially no carbs/spikes!

I regret to say i had to pop a lyrica (50mg) last night. I decided this morning to gently break keto and will be introducing small amount of carbs but I am finallt biting tje bullet and trying the bentyl/dicyclomine I was prescribed in april for dumping syndome. I have to say i took it before breakfast and my glucose isnt rocketing and my intense anxious stomach feeling is NOT there (but was profoundly there this morning). I hope something sticks. I feel like im just spiraling with no way out.

It really frustrates me that the chw group will blame everything on withdrawal. I get tjay my nervous system is torched but these symptoms predated cymbalta and some of us just will NOT get better with a long hold because in my case i am positive that my gut was the issue to begin with. Im still holding but I just cant take this “dont try any supplements ever” heavy handed advice. Sorry a bit of a rant.

Well, firstly i thought something like: "Where the hell did you found out clenching kidneys after mentioning stomach churning?" But it's actually an imaginable and intuitive descriptions, since kidneys are located close to everything else. That's why it's harder to anticipate, where it actually is. But still, it must be somewhere in the epiegastric area according to your thought about GI hypothesis as explanation..

It seems I have said some things that had a lot of undertones that were not intended. In another thread. So I will address those here as well as share an update.

Before my last period cycle I had an extreme worsening of symptoms. I had to take a couple days of pregabalin to get sleep. If i do not sleep I become borderline psychotic from blood sugar regulation issues. I did not take the pregabalin after my period started. My sleep is better now.

The main issue I am dealing with is extreme digestive distress. I have violent reactive hypoglycemia which I suspect was happening prior to the medication as the symptoms were identical. I have a 25 year history of intense digestive issues.

Given that my testing points to my hypoglycemia being an issue of motility and not diabetes I have been taking a small dose of bentyl with my evening (substantial meal). I have successfully avoided overnight hypos and gotten good sleep now for about a week. Combined with an all liquid diet.

I want to be very clear that I have for my entire life avoided pills. I avoided even gettinf the bile acid meds or this one for over ten years because of my feelings about drugs. Givrn that i have slowly become more aggressively malnourished from nonstop watery stool from dumping syndrome post gall bladder removal in 2013, which is now costing me iron and vitamins, I do feel for MY BODY it is right to address the gut motility issue as I do not expect to stabilize without being able to eat.

I do not recommend anyone else do this. For me it is a necessary step right now as the likely “fix” would be surgery. I likely have scar tissue that has been building up.

I have been extremely distressed by both the things I tend to hear from doctors and withdrawal communities. I need to say this as part of my experience - I am physically and mentally disabled. I have autism (which tends toward anxiety and rumination) and long covid, which means that my body is at a deficit for certain things without supplementation. It is difficult for me to deal with the medical community who do not believe my conditions (or withdrawal) exist. It is also hard and anxiety inducing to have to hear that I am going to hurt myself by supplementing things that in my opinion for MY disabled body, are not negotiable. I also have a hard time communicating at times, and feel as if my expressing my personal journey of healing is subject to censorship because it does not fit the mold. I do not mean to suggest anyone else take the route I do. It is my own. And I hope that tne intersection of chronic illness/disability with wothdrawal is better understood at some point. I feel anxious every time I even thinl about being honest about my experience and decisions in my journey. Which is not what I want. I want to be honest about what helps and what does not help so others can have the information they need to make an informed choice.

You mentioned bentyl has psychoactive properties. I have researched this drug extensively (as i do anything I put in my body) and all i can see if that it minimally interacts with the nervous system to slow down contractions in the stomach. (Someone in the old SA forum called it “basically valium” but that seems incorrect as it does not interact with gaba at all) Because I am becoming malnourished this is an intervention I feel is necessary for me and there are no interactions with cymbalta. Do you have any references for its psychoactive properties that detail concerns? If I am missing something I should know. Thanks.

Sorry forgot to reply to this earlier. I’ve no experience or knowledge of this drug other than a quick google search last night. Anything psycho active I’d be cautious of taking.
