You are not experiencing psychosis...this is withdrawal plain and simple.

Each drug causes further complications and suffering. You are way too sensitive to add anything.

@Nickie Evans all that the majority of practitioners know how to do, is throw drugs at the problem until something seems to stick.

This approach clearly has not gone well for you in the past, and you'll find many others here that it has gone badly for.

We don't give medical advice, and cannot advise you to ignore licensed clinicians, but I will remind you that you're also not obligated to follow any particular advice. I'll also point out that scientific understanding of how these drugs even work is very limited, let alone the understanding of an individual person, with a unique mix of drugs in the past, and one clinician who sees you once for a short time.

I'm so sorry your going through this . This exact same thing happened to me after 20yesrs use of Seroxat (Paxil ) they tried me on all different drugs and I ended up on Seroxat with the added Seroquel which I wish I never took .

Please don't take anymore drugs . I know your desperate but please don't . I would do what @LostinCanada and @Chippy said , try a small dose of paroxetine.

I only wish I did things differently when this happened to me .

Dr started me on Esitalopram just over 3 weeks ago, starting at 1mg, and increasing weekly by 1mg, currently on 3mg. Depression slightly lifted, horrific cortisol waves upon waking, cant function until up for several hours. Anxiety bad. Chronic diarrhoea.

@Nickie Evans Please understand that what you are going through is withdrawal from Seraxat and nervous system stress. Many of us in this forum trusted doctors and psychiatrists completely, and sadly a lot of us became much worse because they simply did not understand what was happening to our nervous systems.

Most medical professionals are not trained properly in antidepressant withdrawal and nervous system sensitisation. They often mistake withdrawal for “relapse” or another mental health condition, and then add more medications, increase doses, or make rapid changes that can make the injury even worse.

That is exactly what happened to many of us here.

A lot of us were told “it can’t be the medication” — and later realised it absolutely was.

I was off it for 3 months . I was put back on a full dose but I should've reinstated a smaller amount .

I know too well how scared you must feel I was in your shoes . People here on these forums dedicate there time helping people go through this while there suffering themselves . The last thing anyone here wants to see is good people being harmed by these doctors .

Yes I take in all that the moderators here say and I'm very grateful for their support and knowledge. What dose did you reinstate at please? Then I know to do much lower to start with. Do you stick to 1 particular brand of Paroxatine?

I reinstated way to high 40mg I was desperate . I'm in Australia so I went back to Aropax ( Paroxatine) .

@Nickie Evans like @cheeky I stopped paroxetine completely from 30 mg and after 3 months I reinstated 5 mg which was too high. I had found survivingantidepressants.org at the time but they weren't allowing new members so I just guessed at the dose. I was told once I got on the site, that 1 mg would have been the better reinstatement dose.

It is not a perfect solution but better than the alternatives.

Saw psychiatrist this afternoon, he asked lots of questions and listened to me and heard the hell I'm in these past 5 months. Initially said venlafaxine, I questioned why not Paroxatine, he said because he thought i may be too scared to take it due to events in early January. I asked would Paroxatine have been his first line of choice he said yes, so agreed reinstating Paroxatine, he said 10mg but I will have the tablets. Slightly concerned as I only have 3 Mitazapine left and he wouldn't prescribe anymore so I could slowly taper off it, was going to suggest a cross over, he said take 7.5mg for the next 3 nights and then stop and start Paroxatine the following morning. I live in hope 🙏 please don't say anything negative about the way hes told me to come off mirtazapine as I have no option as he refused to give me anymore when I requested a further supply so I could slow taper off it. I'm fully aware its not an ideal situation to just stop mirtazapine at 7.5mg and start Paroxatine but I have no other option.

Thank you for your support these past few days, I really am at rock bottom so please don't say anything that will cause me any more anxiety than im already struggling with. I know you all would have suggested a slow taper off mirtazapine and I understand that would have been the most safe option.

Meant to say i will HALVE the 10mg tablets. Not have.

Hi @Nickie Evans I am glad he saw the need for paroxetine. Are you going to get a scale and weigh the 5 mg. I do strongly suggest this for even dosing? If you have a negative reaction within 2 hours of taking the 5 mg, you may need to go down in dosage. We can help with that. 😊

As far as the mirtazapine goes...I would make those pills last a bit longer...again just by cutting the dose in half for two of the pills and then cut the last pill into 4 pieces....

I’m so very sorry for how things have been.

I’m glad you have a supply of Paroxetine.

I agree with @LostinCanada come off the mirt as slow as your supply will allow before stopping.

I would highly recommend you start on a much lower dose of Paroxetine. 1mg would be my suggestion.

There are 4 reasons.

1. You’ve been off a while now

2 You reacted to generic Paroxetine before and that reaction will likely be lower at a lower dose

3 Low doses of ads are very strong still which I know is hard to believe but they are. 1mg is about 30% effect on the brain and we have seen this is enough to stablise people.

4.People very often react badly to high dose ri especially this far out.

You could use the next few days to get 10ml and 1ml needleless syringes and we can help you make a lower dose as a liquid. You don’t even need to tell your doctor you are doing this. Tell him you take 10mg or 5mg.

It’s better to go up than to have a reaction and have to go down.

We have seen many stabilise on a low dose and been very pleased they did so.

In a hyper state of anxiety. I'm so frightened that reinstatement won't work and I'll be stuck like this for ever. Knowing my history how confident do you feel that reinstatement will help me?

14/03/26 Dr advising to start Esitalopram 5mg to help with current situation. (Not started, waiting advice from people here first) 

Dr started me on Esitalopram just over 3 weeks ago, starting at 1mg, and increasing weekly by 1mg, currently on 3mg. Depression slightly lifted, horrific cortisol waves upon waking, cant function until up for several hours. Anxiety bad. Chronic diarrhoea.

I wish we could be certain of things.

We can’t know how reinstatement will affect anyone. However I think 1mg has a good chance to settling things for you. I do worry about you having a higher dose. We have had lots of people reinstate low doses of their original drug and it stabilise them.

You won’t be stuck forever. The body is amazing and you’ll feel better eventually. Ri is a good option to cushion all this for you.

Thank you for your encouraging words, I not only obviously want and need to be in a better place for myself, but these last 5 months have had an impact on my daughter and my mother, my daughter has seen me in such a horrific state multiple times and it does effect her understandably, I was always a strong, supportive mother, who now is a crumbling mess. I was carer to my mother, who now has had to leave her home and go into respite care as i am no longer able to do all I did for her this time 6 months ago, my mental health, all because a medication stopped being manufactured has had a dramatic knock on effect to them also 😓

The psychiatrist reasoning behind starting at 10mg is that will slightly combat the withdrawal from the Mitazapine, I do feel however a lower dose is a safer option and will be discussing this as soon as I am able to get a response from the mhct.

I can completely understand that. We have all had this too. The knock on effects on our families is big.

This proves if you have any doubt that this is all due to the drug harming you.

I can understand the theory. But I think in practice I don’t agree that’s a good idea. I think you are risking reacting to the high dose. I think coming down on the dose of mirt as slow as you can. Maybe start the Paroxetine before you finish mirt completely. But the drugs work ok different receptors anyway. Mirt works on multiple receptors and is a very different kind of drug.

I suspect they won’t understand the low dose. Maybe they will but I think this is something you’ll have to choose to do for yourself.

@Chippy I only have 2 and a half Mitazapine left, the psychiatrist refused to give me a prescription for anymore when I asked could I taper slower off the Mitazapine. The tablet's are so very small, halving them is successful but getting a quarter isnt, as it just breaks unevenly or disintegrated.

I understand. You can only work with what you have. If you had scales you could weigh the pieces which is what we recommend. But in reality you don’t have a lot of mirt there to play with so it’s not much of a taper obviously.
