Update; Mark Horowitz told me to updose. Its now 1 month on 3 mg. He told me watch the symptoms and slowly updose until I feel some how fine. I don’t know if I’m doing the right thing. He thinks it will help me because I had relief from the 10 mg earlier.

Thanks for the update. It is great you spoke with Dr. H again.

Hi ,

Increased to 4 mg yesterday but had an active reaction so went back today to 3mg. Will I be ok? I feel so odd and disconnect from live. Had a few days last week when I felt relief. Can someone tell me if I’m doing the right thing. Feels like this is my life now in bed most of the time . Will I ever stabilize? Mark also doesn’t know what to say. He just told ne to increase because the last Ri did me good but that also could be a “honeymoon period”. So he told me I could gradually do up. But it’s a risk. I don’t know what to do anymore. Has anyone have experience with Ri and how to handle this situation? I’m going to hold now for a while on 3mg. Anyone who can help wirh experience?

Just wanted to highlight this post for you where you felt 4mg was activating.

If you choose to increase your dose I would personally be cautious at first because of this reaction here.

Let us know what you decide to do and how you get on. Thinking of you xxx

Thank you chippy. When will you notice a bad reaction? Did you ever saw someone getting better eventually by updosing slowly?

01-2018 fluoxetine 10 mg two weeks

02-2019 started Sertraline 150 mg with 10 sometimes 5 mg oxazepam for sleep- my sleep was not ok after 150 mg but I felt good

01-August 2025 in one moth quit Sertraline because of the symptoms (gaining wait, vertigo, low mood)

12-2025 quit oxazepam

04-2026 because of major brain zap RI 25mg one week, up to 50mg next week and get depressed and quit. This was a high dose.

I also started oxazepam again

17-Mei 2026 RI 10 mg 3 days and 5mg 2 days in the hospital.

We always suggest not jumping doses around. But if the reaction is severe return to the previous dose. Otherwise hold the dose for 7-10 days. Sometimes thing can get a little worse. This could be just what happens as you increase but could be waves and windows playing out anyway.

People definitely can benefit from updosing after ri. We pretty much always have people on an initial entry dose after ri and then slowly ease it up. Much safer approach.

Thinking of you @Saar8srl. I'm just going to briefly summarize our chat from the dm's here so that everyone's aware of what's going on and the advice that has been given.

You met with Dr. Horowitz again recently and he recommended that you continue up-dosing slowly. We talked about how you did briefly feel better on 10 mg in the hospital back in May, but you also had a minor activating reaction when you recently tried to go up to 4 mg.

I said that there probably won’t be a solution that will fix everything, besides lots of time as we always say. Up-dosing could help if you felt better previously on a higher dose, but often we can’t recreate how we felt previously just by going back to a particular dose because our nervous systems are so sensitive now. So there’s always a risk.

I recommended that if you do want to try up-dosing again to go slowly, like try 4 mg again for a week and see how things go.

Please let us know what you decide 💜

It’s good things are not worse immediately so I’d concentrate on that, so giving it time will allow you tell the effect. Maybe it will be nothing. Perhaps you’ll feel a bit easier. Then you can choose. Continue to increase or not.

The question is do I help my brain heal with a small Ri or does it just do nothing. I dint know that yet. I have to wait and see what the 4 mg is doing . If the depression and my mood will get a little better I will be glad . Since I updose I see more windows tbh . But I don’t know is this is the Ri or the time . The depression is at least not so deep and the DPDR is a little less. But it’s just hard to say because I’m mostly in a wave and I don’t trust myself in a wave …

You should tell us how things are going. All the good bits not just the bad. The good bits give an indication of improvement. The fluctuations are signs something is changing. This could be ri helping. To can take many weeks to show its full colours. So long as you don’t get lots worse it’s ok to increase the dose. Just carefully. No panic. Like you said. Waves make you not make good choices potentially.

Maybe do a daily diary. Just a little how you were that day with the good and the bad in a time order. You may have seen others do it
